BEFORE YOU BEGIN:

You will need the following information, for each location performing drug and/or alcohol test collections.

• Site Name
• Site Address
• Telephone number
• Primary Contact -- Name and e-mail
• Name of each collector to be registered at each location

(Single collectors can be registered at multiple Collection Sites.)

The registration process allows you to conveniently pay for your location(s) in a secure online transaction with multiple options:

For Credit card transactions:
   Credit card number 
   Billing address of credit card
   CCV number on the back

For EFT or ACH online transactions:
   Routing number
   Account number 
   Bank address
   ACH check number

If you choose to pay by check, simply print an invoice upon completing the registration process and send to:
